Do you use the ForwardHealth Portal? We want your input! The ForwardHealth Portal is a key tool for Wisconsin Medicaid providers and partners, and we want to make it the best it can be.

Please take a few minutes to share your honest opinion about the ForwardHealth Portal by June 11. Take the ForwardHealth Portal User Input Survey, or access it at the top of any portal page.

We’ll use your feedback and ideas to improve, adjust, and add to the ForwardHealth Portal. Our goal is to make it easy to use, quick to navigate, and as helpful as possible for all site visitors.
